
Welcome Dr. Carly Coulthart
Hi everyone! I’m Dr. Carly Coulthart, the new sports physical therapist here at Streamline Physio. I’m excited to join a team so passionate about helping active individuals move better, perform stronger, and return to the activities they love.
I grew up in Sun Prairie, WI, where sports were a central part of my life from an early age. I competed in basketball, cross country, and track in high school, with prior experience in volleyball and softball. My passion for physical therapy comes from navigating significant injuries during my own athletic career — experiences that fuel my commitment to helping others stay off the sidelines.
Outside of athletics, I spend a lot of time with family and friends, which is a big reason I wanted to come back to the Madison area. I’m excited to be more involved with my nephew and my niece who is on her way. I also have two fur babies, Tito and Finn, who keep me on my toes more than you’d probably expect from cats. I try to enjoy the outdoors as much as possible with camping, water sports at the lake, traveling, and attempting to work on my golf game.
My path in sports rehabilitation began at UW–La Crosse, where I earned my Bachelor of Science in Exercise and Sport Science and my Doctorate in Physical Therapy. I also had the privilege of competing for the women’s basketball team, an experience that influenced the way I understand and approach injury, rehabilitation, and performance training. I understand the environment of competitive athletics and the value of clear communication, personalized plans, and working with a clinician who truly understands the demands of each sport and position.
Following my time at UW-La Crosse, I completed a Sports Physical Therapy Residency at Aurora BayCare Medical Center in Green Bay, WI. My residency allowed me to work with athletes across all levels — from weekend warriors to elite competitors — and strengthened my skills in return‑to‑sport progressions, movement analysis, on‑field injury management, and performance‑based rehab. I am especially passionate about working with athletes returning from ACL injury, overhead athletes managing shoulder pain or instability, and both recreational and competitive runners.
As a PT and Certified Strength and Conditioning Specialist, I focus on building programs that not only help athletes recover, but elevate their strength, power, and confidence as they return to sport. I believe in evidence-based practice and treating the whole person, building a plan of care that fits their goals and promotes long-term health.
